Why do windows come loose?

Aircraft windows are designed to withstand extreme pressure changes at high altitudes. A window coming loose, as in the Ryanair incident, is highly unusual and serious. Investigations will look at manufacturing, maintenance, and potential fatigue of materials, as even tiny flaws can be critical when a plane is under stress during flight.
