TfL wrongly fined Ulez driver despite payment failing on its end

An elderly driver was threatened with escalating fines and car destruction after Transport for London (TfL) failed to collect a £12.50 Ulez payment. The driver, in his 80s, paid the original charge on his American Express card but TfL's contractor didn't collect the money, then rejected his appeal. TfL has since apologised for its administrative error and issued a refund after the issue was challenged.
