Southport Attack Could and Should Have Been Prevented
The Southport attack, which killed three children and injured ten others, could have been stopped. A public inquiry found "catastrophic" failures by Axel Rudakubana's parents and various agencies meant clear chances to prevent the July 2024 murders were missed. Inquiry chair Sir Adrian Fulford noted a "merry-go-round" of referrals meant no agency accepted responsibility for the grave risk.
