Why does heat cause cancer?

The exact mechanism isn't fully understood, but it's thought that repeated damage to the lining of the oesophagus from very hot liquids could lead to changes in cells over time. This chronic irritation might increase the likelihood of cancerous cells developing. It's a bit like sunburn, but on the inside.
